What to Expect
Thank you for choosing DePaul Community Health Centers. To deliver the best care, our dedicated health care professionals work with you to develop plans for both treatment and preventive care.

Your Care Management Team includes a Provider, Care Coordinator, Nurse, a Pharmacist and/or a Social Worker.
During your visit, you can expect to see a doctor who leads the health care team. You can also expect to see a Care Coordinator who will arrange for you to receive tests or services not provided by DePaul. If your doctor recommends a test or service not available at one of our health centers, your Care Coordinator will arrange for you to receive tests or services at another facility.
If you need help managing a chronic condition, your doctor may assign a Care Manager who will:

Give you information and tips to help avoid complications

Teach you new skills to help manage your condition
Before Your Appointment

Confirm the date, time and location.

Make a list of all of your medications to share with your doctor.

Locate your most recent insurance card and prepare to bring it with you to your visit.

Plan to arrive 30 minutes early.

In the event that you can no longer keep your appointment, contact us at least 24 hours in advance to reschedule.
While You’re Here

Provide our courteous front office staff with your most recent address, phone number, and insurance information.

Confirm the date, time and location of your next appointment.
Emergency and After Hours Care
Please call 911 or visit the nearest hospital emergency room in the case of an emergency.
For non-emergency assistance outside of normal operating hours, please call (504) 207-3060 for the on-call physician 24 hours a day everyday.
How to Pay
Health care is not our job. Health care is our ministry–body, mind and spirit. That is why DePaul provides discounts to uninsured patients based on the size of your family and your household income. In order to comply with federal health care regulations and guidelines, we are required to collect proof income from all patients who wish to qualify for our sliding fee scale.
You qualify for a discount if you meet the income requirements and provide proof of insurance, Medicaid, or Medicare. Please note that DePaul Community Health Centers is a Medicaid and Affordable Care Act enrollment site.

You may qualify for a sliding fee scale based on your income or ability to pay. If so, you are required to pay between $35 and $95 for medical and behavioral health visits.
For dental visits, there is a $55 fee for your exam, x-ray, and treatment plan. This applies if you are a self-pay patient or uninsured dental patient who qualifies (household income between 100% and 200% of Federal Poverty Guidelines).
Payment is expected at the time of the visit. Additionally, all reference labs and invasive procedures have a separate fee that must be paid prior to the lab test or procedure. Please note that reference labs are labs that we send to an external company (such as Labcorp or Quest Diagnostics).

Payroll Check Stubs (1 to 2 most recent, must be within last 30 days)

Benefits Checks (Food Stamp Budget Letter, Unemployment checks)

Bank Statements (3 monthly Statements-Must show company name or consistent deposits)

SSI Check/Retirement/Award Letter
