Patient Privacy Notice
We take patient privacy very seriously, and it is important to us that our patients are made fully aware of a potential privacy issue. Regrettably, we have learned that personal information associated with a consent form signed by parents of a school-based program on October 6, 2017, may have been disclosed to an unauthorized third party.
We understand that this may pose an inconvenience to you. We sincerely apologize and regret that this situation has occurred. DePaul Community Health Centers is committed to providing quality care, including protecting your personal information, and we want to assure you that we have policies and procedures to protect your privacy. We are taking necessary and appropriate steps to prevent this type of incident from occurring in the future. Specifically, we are working with our employees who travel to various work sites to create further safeguards in transporting patient information and retraining appropriate staff members to ensure that this never occurs again. We will also be reporting this incident to the applicable authorities as is required after a disclosure of protected health information.
